Well Child Appointment Cancellation/Rescheduling policy:

We kindly ask that you give us at least 24-hour notice when canceling an appointment. Failure to do so will result in a $25 cancellation fee.

Sick Child Cancellation policy:

Please call us at least an hour before your appointment time to cancel your appointment as we could use the time of your missed appointment for another child in need of a visit.

15 minute grace period:

We understand that sometimes things happen to delay your day (especially when children are involved) but please be courteous and call us if there is a chance that you are going to be 15 minutes or less late for your appointment. Anyone arriving after the 15 minute grace period will automatically be considered a “no show” and a $25 fee will be due.

School & Camp Forms policies:

 

• Please allow 7-10 business days from the time of drop off for us to fill out the forms. This is free of charge.
• We will happily call to inform you when your form is ready for pick-up.
• If our schedule allows it, we will be able to expedite your forms and have them ready within 24 hours for a $10 fee.
• Forms are considered expedited and the $10 fee applies for any form requested to be returned before our minimum 7 business day period.
• If you requested your form to be expedited, it is not guaranteed that you will receive a call letting you know that it is ready.
• We will only fill out forms for children who are up to date on their yearly well child visits. If you have any questions as to when your child’s well visit is due, please feel free to contact us at 703-858-0121 and we will be happy to help you.
• If more than one form per child is to be filled out within a year of the last well child visit, a $10 per form fee will apply to each additional form.
• If you would like us to mail your form, please provide a stamped and addressed envelope.

Referral Policies:

Specialty referrals will only be given when ordered by Dr. Davis. Dr. Davis must be listed on your insurance card if the referral is to be valid with your insurance company. Please call our office AFTER you have scheduled an appointment with the specialist that Dr. Davis recommended. Please allow a minimum of 7 business days for us to complete a referral. Please do not see a specialist without a referral.

Immunization records and Growth charts Requests:

Immunization records and growth charts can be provided to the guardians upon request and free of charge.
